There are several hundred people behind the scenes. They must have directors, sound people, camera crew, and medical crew for both tribes, along with other production staff. They have service people, like cooks (for the TV crew), and builders ( for the challenges). They have people who try out every challenge before the contestants actually do them. These people are well hidden from camera, but not that far away.
Sometimes the contestants re-do and re-act scenes so that the cameras can get it from different angles. That part is fake. They even use stand ins sometimes when they do this. Sometimes they are asked questions that make them say certain things that we see during the interviews. That could be fake as it appears they are saying it on their own.
They are not given food by any of the staff, so they do have to find their own food, and they do build their own shelters.
