#1AvrilFAN is, for the most part, right about how he came into being although the only "flesh" parts of him are his head in the helmet and the gross looking sac that Obi-Wan shoots when he kills him. That "sac" contains Grievous's heart, lungs and such. However, that is not the reason he is coughing throughout the movie. If you watch the Clone Wars cartoons from the Cartoon Network you will see the reason at the very end of the last episode from season two. That whole episode is about how Grievous attacks Coruscant and kidnaps Palpatine. As Grievous is about to board his shuttle to return to his flagship with his hostage Mace Windu arrives. Mace attempts to use the Force to crush him ... kind of like we see Vader do to his officers who have "failed him for the last time" ... Mace attempts to crush that "gross sac" that Obi-Wan shoots to kill him and in the process damages his "lungs" ... that is why he is coughing throughout the movie.

Because He was in an accident and they had to reconstruct him he used to be flesh but now he is half alien half droid. The separitists told him they would reconstuct him if he would be their general and then he had lightsaber training from Count Dooku.
