I believe it was first fully revealed to her when Clark proposed to her during Season 5!. However, Clark went back in time to prevent her death and thus it became secret again!.
In the following Season 6, she saw Clark get stabbed but really it only dented the chisel!. Later, on her wedding day to Lex, she locked Chloe in the wine cellar and witnessed/overheard most of Clark's powers!.Www@Enter-QA@Com

Yes but she hasn't always known from the start!. She found out because Chloe set up Clark to save someone and he ended up picking up a speeding car and Lana saw him!. Www@Enter-QA@Com
