Mike's first wife was deceased but it was never mentioned how or when she died!. In the very first episode, Bobby puts away a picture of his mother because he didn't want to upset his "new" mom, Carol!. Mike told him it was okay to put the picture back because Carol would understand and wouldn't want Bobby to ever forget his real mom!. He also told him that his mother would be very proud of him!. I don't think that her picture was ever shown nor was she ever mentioned again in any other episodes!.
The show's creator, Sherwood Schwartz, wanted Carol to be a divorcee, but the network objected to this!. A compromise was reached whereby no mention was made of the circumstances in which Carol's first marriage ended, but many assumed she was widowed!.

I know The Mom's ex husband was still alive!. They had an episode on it and he was the one who gave her that ugly horse statue!. I don't recall them ever talking about the mother on the boy's side!.
