Yeah, Jack Bauer's wife dying in season one of 24 was pretty awful - I mean, after everything they went through in that season and at the very end she gets killed - poor Jack!
My most awful, terrible, painful moment on a tv show has to be the final scenes of the season 3 finale of Supernatural when Dean gets killed!.
All through the season they were telling us he was going to die but they kept giving us little bits of hope all the time that there might be a way to save him so right up until the last moment, I'm thinking, he's going to get out of this somehow, somebody's going to save him but then he literally got torn apart by hellhounds in front of his brother Sam and then wound up being tortured in a sort of Hellraiser style Hell!.
Oh! I cried my little heart out over that one :*(
Edit!. I just read Shadow Kat's answer and I totally agree with her choice of The Doctor and Rose being separated in different dimensions in Doctor Who - I still can't help shedding a tear every time I see that episode! :*(Www@Enter-QA@Com

If i hadn't seen the 4th season of Doctor Who, i would've probably said my most painful moment was (Season 2) "Doomsday"s final scene , where i felt sooo sad & almost cried for both rose & the doctor, BUT,
After watching "Journey's End" , i CAN'T FORGET The (Real) Doctor's face when he was leaving rose, who was kissing his clone!!
I feel for him!.!.!.!.!.!.
Rose got her loved one & can live with him for the rest of her life, but the (real) doctor, not only lost the love of his life, but also had to suffer seeing her being kissed by somebody else & knowing that she will live happily w/o him ( sth he can't do i believe!)
That was sooooooo cruel ! (curse you RTD for doing that to My Lonely Doctor!! )

Doctor Who, series 2 episode "Doomsday," when Pete appears and takes Rose to the parallel universe and then later when the Doctor uses the energy from a dying sun to project a hologram to her to say goodbye!. I can't watch that episode without the tears falling!.
Torchwood, series 2, when Owen is at the nuclear plant and is ranting and raging about being trapped and really dying this time, and Tosh back at the Hub asks him to please not be angry and Owen says, "Why not!? Why shouldn't I be angry and scream!?" And Tosh says, "Because you're breaking my heart!." And then of course, Tosh dies on screen in a gripping and intense scene and Owen's communicator signal blinks out on the screen monitoring the nuclear plant!.
I'm sure these aren't the only moments that tore me in half, but they're the most recent and so the most vivid!.Www@Enter-QA@Com

Oh wow, SO many! Probably either when Lorelai married Chris in Paris on Gilmore Girls, or when Rory and Lor weren't talking because Rory stole a yaht with Logan!.
Or on Bones, when the episode ended with Booth getting shot and then the next episode when it was his funeral and finally like five minutes in did we realize the funeral was fake!. MEAN!.
Or on NCIS, when Kate got shot!. That made me start crying!.
Or on Veronica Mars, when Veronica thought that Keith was dead when Beaver blew up the plane that was supposed to be carrying Keith!. Or even when the time Veronica was almost raped again and killed, or when Logan's dad tried blowing her up with a match and gasoline!.

this is quite sad but in the lion king when mufasa dies trying to save simba and then little simba trying to wake him then starts crying and cuddles up under his paw always makes me cry I'vee seen it so many time and i know exactly whats going to happen but i still cry

First that came to mind was in The Office when Jim confesses to Pam that he loves her and she rejects him (twice) and he's so heartbroken and even cries a little :( It was so sad!.
