The Doctor telling someone his real name is amazing!. He must have really, really, really, thought a lot of River Song to tell her his 'actual' name!.
But if it was whispered in the Doctor's ear and the audience didn't hear it then don't expect to know it!. No one knows the Doctor's real name and if they did it would kill the mystery out of it!. I have read on one website that the Doctor once stated that his real name would be hard or difficult for a human to pronounce but has the same syllables and/or pronouncing as 'Who' at the beginning of his name!. so that is where we get the idea of having 'Who' in the title of Doctor Who!.
There is also this idea that Gallifreyan names are long drawn out affairs, that are difficult for people to say or use!. This isn't always the case as there are a few short Gallifreyan names in the history of the show, but a primary example of this is Romana's name, as Romana's full name is Romanadvoratrelundar!. I would imagine that the Doctor's real name is something like this!. Something long and difficult to pronounce!. But this is all speculation on my end!.
Personally I wouldn't want to know the Doctor's real name, it would take away the mystique that makes up part of the show and then it would become boring!. I like the whole 'I don't know his name' but he calls himself the Doctor!.Www@Enter-QA@Com

The Doctors name has never been revealed, in fact the character never started out as the Doctor that was introduced during the very first episode, it seems to be something that is widely debated on Internet forums/chat rooms at conventions etc, but it is something that is plainly not known even the great Russell T Davies hasn't got an idea for a name like he says whatever you decide it would only be a disappointment so it works better by leaving it to the individuals imagination!.Www@Enter-QA@Com
In the yrs I've watched Dr Who, I never heard his full name!. Just the Doctor or Dr Who!.
