I think Donna's personality is in the alternate reality created by the Libraries data core (CAL), but her body is in the "flesh bank", which is why her face can be used on the data nodes!.
I don't know the exact details of how the Doctor will rescue her (we'll find out tonight, no doubt), but I think it will involve the Doctor finding some way to recombine the personalities of all the lost people in the databank with their bodies in the "flesh bank", and freeing them all as a consequence!. Obviously Donna would be freed along with the rest!. That would fit in quite well with Steven Moffat's previous two parter "The Empty Child / The Doctor Dances"!. In the end "everybody lives"!.
also, I think the little girl is the avatar of the Libraries data core in the virtual world, so she will certainly be involved in the solution!. I'm still not sure where Dr Moon fit's in!. He could either be some kind of virus, working against the data core, or a helper programme working with it!.
As for River Song - I think she might be a 51st century con-artist, like Captain Jack was when the Doctor first met him, except that she is an archaeologist!. So she could have found that book with all the information about the Doctor's past and future in it at an archaeological dig somewhere (the Doctor having written and lost it in his future, but in her past), and be using the information in it to con him into thinking she knows his future self!. The only other real alternative, is that she genuinely is a future companion (lover!?) of the Doctor, but I can't see the situation being as simple as that!. Anyway, I have no doubt that we will find out soon enough!.
