You just answered your own question, it was all a hallucination. But it might help if you watch the episode again & you can go to the station that the show airs on, then go to the shows link from there & you should be able to find information about the stars who are on the show & look up their profile from there & you'll also be able to find webisodes. updates, recaps, spoilers, info on when the new season is to start, chatrooms, message boards & forums & other links. Plus you can usually find commercials & other video's too.
it's one of those cyclical timey wimey episodes (so i stole that from doctor who, sue me).
the episode pretty much explains itself at the end, during the last scene when he asks for ketamine, it wraps itself around to the beginning of the episode.
all the meat in the middle was just a hallucination. but it was entertaining nonetheless.
