When Alexis was a girl she was in boarding school. She got knocked up (the writers never told us by who) and her father Mikos Cassadine, made her give it up for adoption.
When Alexis was older she went to search for her child only to find that the trail led to a little girl who was horribly killed as a child by a car accident (Car hit her). Turns out that the trail had been fabricated by Mikos so that Alexis would beleive her kid was dead and would stop looking. And thats what she did.
20 odd years later....Sam discovers her mother was not her biological mother. Sam and Jason investigated and it led them to the discovery that Sam was actually Alexis' long lost child.

Alexis was too young to have a child and her family would not allow it, they would have taken Sam away and put her through the hell that Nicolas went through. She was doing what was best for Sam at the time.

Although Alexis also felt it was for the best, as she was only a teenager, she was still heartbroken at having to give up her baby and went to search for her, after she recovered from being sick from the deadly virus epidemic in Port Charles a couple of years ago. She was lead to believe her baby died, but later discovered from Jason, when Sam was shot, that Sam was her daughter.
