Everyone can hear Brian, whose - unfortunately - the show's voice of reason. As for Stewie, this is what I found on IMDB.

If the joke/scene requires people to hear Stewie then they hear him, otherwise what people hear (besides Brian and other babies/children) is just considered "baby talk"

the family can obviously hear them talking if they answer back.

They cannot hear Stewie (the baby), the dog can hear him but the rest of the family cannot.

everyone including the family, friends, and just everyone, can hear them talking. Stewie (the baby) is for the most part ignored, or taken sarcastically, while Brian (the dog) is regarded as just another member of the family.

Although at times it may seem that only the dog hears Stewie, everyone really does. Brian is the only one that responds for the most part, except for in select episodes. (ex: family guys 100th episode, where stewie asks his mother, could you really kill your baby? and she says no)

When the baby talks, the dog can understand it clearly, but the humans kind of translate it to baby talk.
As for the dog, everyone understands it exactly. he's like a human with fur.

brian is heard by all

Stewie is a switch between heard and possibly just ignored....
