Dorian is Adriana's biological mother. Dorian was briefly married to the Santi guy and had Adriana. When they broke up Adriana went to live in a convent and led to believe that another couple were her real parents. Antonio's real father was the Santi guy and Carlotta is not his real mother. She is Santi's sister making Antonio her biological her nephew. And Christian is Carlotta's biological son and raised with Antonio as his brother by Carlotta. That makes Antonio and Adrianna half siblings and biological cousins to Christian.

I think it was in '03. A relative of Dorian's named Aunt Betsy said she was dying and wanted to leave everything to a Cramer woman who had not been in a relationship. So Dorian concocted a plan to make it seem like Dorian had given up Adrianna. Dorian made sure Aunt Betsy found out about Adrianna so that Adrianna would inherit everything from Aunt Betsy, but Dorian would be able to spend that inheritance. Eventually, I don't know how, it was revealed that Dorian really was Adrianna's mother.

Upon Adriana's re-arrival in Llanview, Dorian Cramer Lord announced that she was her biological daughter and that Adriana, meanwhile, was to inherit $30 million dollars from Dorian's Aunt Betsy. In spite of Dorian's revelation to being Adriana's daughter originating as a scheme, cooked up by she and David Vickers to secure Betsy's money for themselves, it was later proven that Adriana was indeed actually Dorian's biological daughter.
NOTE: Adriana being Dorian's daughter was originally meant to be a hoax. The storyline, however, was switched midway through when OLTL's producers recognized a resemblance between Robin Strasser and Melissa Gallo.
Dorian is her biological mother. She had a secret marriage to Manuel Santi and produced ADrianna, she was sent by Dorian and Carlotta to live with friends. Manuel is Carlotta's brother, he is Antonio bio dad, Carlotta adopted him at birth. Cristian therefore would be her 1st cousin and Antonio her half brother, and Carlotta her natural aunt!
