My guess is that the guy in the video that Ben showed Locke was spying for Ben and collected the info Tom gave to Michael. OR we could go completely the other way and say Ben faked all the documents and is responsible for the staging of the plane. I think the first is more likely.
no, whitmore was taped, and he also told some people, because at least someone had to know that all the people on the island were to be killed. snd some leaked, of course.

Ben had people spying on Widmore if you remember the previous episode Ben showed John Locke the tape that his spy filmed of Widmore.... so ben is keeping his tabs on Widmore the same way Widmore is keeping tabs on the island

At this point, it's hard to tell who really staged the crash. If it were Widmore, it would be no thing for Ben to get pics. He has people everywhere. It's anyone's game at this point. You can't trust Tom. Ben probably staged it.
I remember someone on the show saying that Ben had staged the whole thing. Since Tom is one of his minions, he could easily have the info. I'd bet on Ben being behind it before Widmore.
