Bo's (Shawn's dad) real father isn't the Shawn Brady who owns the pub, it's Victor Kiriakis.
Victor and Kate had Phillip
So Bo and Philip are half brothers, so Phillip is Shawn's half Uncle.
Belle and Shawn are no way related, well, only by marriage.

Years ago, Caroline, Victor and Shawn (Grandpa Shawn) were friends. Caroline was in love with Victor Kiriakis but Victor's father felt that his son needed experience in the family's shipping business and sent his son to it's home office in Greece. Caroline was heartbroken and eventually fell in love with Shawn Brady. The two married and had their children Roman, Kimberly and Kayla. With Shawn away a lot, Caroline discovered that Victor had returned to Salem and they soon began an affair. This affair produced Bo but no one but Caroline knew the truth. She had kept the secret for years (I think Bo was in his early 20's) until Bo, who was then a police officer, had a gun pointed at Victor's head, threatening to kill him. "Stop! He's your father!" was what stopped him from pulling the trigger. Everyone was shocked to learn of the affair but Shawn, being the loving, forgiving man that he is, of course, forgave Caroline and the two mended fences.
Victor has had many women in his life...his first affair produced Isabella Toscano (she was married to John Black (nephew of Shawn Brady, cousin of Roman, et al.) and died of pancreatic cancer right after soon after their son, Brady, was born), Caroline Brady (their son, Bo), Kate Roberts (their son is Phillip).
So, to make a long story short...
Bo and Phillip are half brothers.

Belle slept with and was married to her present husband's uncle.
And, for the kicker...Belle is more (bloodlinewise) that Shawn is. Yikes!
To answer your question...no, they don't have the same grandma. Caroline is Shawn's grandmother but she is Belle's great-aunt (Grandpa Shawn is John's uncle). Since they are all so close, she call's her grandma. <G> What's so cute is that in a Shelle fanfic I wrote years ago, "The Rebellion", Marlena verbally attacks Belle for calling Caroline "grandma". Interesting.
