I thought I heard the American voice over guy say that one of the competitors injured their shoulder from falling off one of the obstacles. There was even a video clip of him being carried away from the course.
It's actually pretty safe. Barring a catastrophic fall from the spider walk or the fourth stage or something like that, most of the course is well padded and the water is there to break their fall.
