Pinky is crazy because he's off the wall, and Brain is insane because he's a mouse who is trying to take over the world! Although the song says "one is a genius, the other's insane!", I suppose they just used that word to imply crazy -even though it has a different meaning- because it sounds better.
Brain is the genius...he was the one genetically modified to be smart, hence his larger head. Pinky is more of a "normal" cartoon mouse...kinda goofy, so he's the insane one because he bothers Brain so much.
I loved that show back in the day!
