I just did a survey question on that with this question as the bonus question...I agree with somebody that answered their parents. If they are alive now and in your past, they are your safest bet and easiest to make contact in both times.
What if you got shot back to when you were 4 years old, that would suck because you probably couldn't comprehend anything going on, but your parents would be available at both times.
I guess your next safest bet would be a childhood friend you may still keep up with.

I haven't given one thought to it. If I had to I'd just pick someone in my family. Desmond didn't have that choice b/c he didn't have family and was pretty much a loner.
