I think you're thinking of 'St. Elsewhere', which starred Denzel Washington (I think).
At the series end, it turns out that the whole show was just part of the imagination of an autistic boy; the scene ends with him looking at a snow globe.
It was called St. Elsewhere :D The episode with the snowglobe was the series finale.
