The show has fast forwarded 4 years. The producers decided that "teen dramas" die when the kids go to college, so they decided to skip it ahead. This first episode was them revealing basic elements of where the characters are. Apparently they will be doing lots of flasbacks of the missing 4 years. Lucas wrote and published his novel. He is now back at Tree Hill High coaching the basketball team with Skills (the team is in horrible shape). Hayley is now a teacher at the school. The night before the NBA draft, Nat got into a bar fight and thrown threw a window and took a shard of glass to the back (hence the wheelchair). Peyton went to LA and is still an assistant to the assistant for a music studio. She decided to move home. Brooke's fashion line "Clothes Over Bros" is a huge success. She gets a distressed call from Peyton, mets up with her in Tree Hill and it looks like she is moving back. Mouth is trying to get a job as a sportscaster. One of Hayley's students is disrepectful to her, Lucas isn't happy with that. Turns out the student is the only good basketball player. Lucas cuts him from the team. Also Lucas is dating his editor, who is pressuring him as Lucas is late in delivering his next book (he has writer's block). Him and Peyton broke up, there is more to be revealed on that.
Probably more of a summary then you needed, essentially the main point is that it is 4 years later, they've graduated school and somethings were revealed and some will be revealed slowly.
