well stefano did get the people in salem to believe that john was roman brady so they could be alerting his appearance for a different actor(like they did with philip)
one of the web-site rumors is that this nemesis stefano is talking about is johns mother that leads me to believe that john is the patient..hopefully it wont take months to find out like it did when victor(and steve) was/were holding the pawn aka roman brady aka john black all bandaged up for months
