A relationship takes two people. I think that they are both at fault for the breakdown. Jack should never have gotten married if he was not going to commit time to their union. However, Sarah knew Jack is a doctor. That means him being on call. Sarah would not be walking if it Jack had not given the time and care to her as a patient - the very qualities she is complaining about now.
I didn't see her that way. Jack had become the distant one due to his being absorbed in his work. Remember the episode in season where Jack was attempting the miracle surgery on the Italian man and had an attraction to the patient's daughter? As Sarah mentioned, Jack needs someone or something to fix. Sarah realized she would always lose out to Jack's career.
