The bomb was a double episode as was Denny's death, as was Meredith falling in the river when the Ferry Crashed and when George's father died. they were all intense but you can't pick one episode without picking two or three in that situation.
But as single episodes go last years finale episode was the most intense in that George failed, He found himself in love with Izzie, Burke professed his love and then left Christina at the alter, Meredith broke up with Derek, and Callie was made Chief Resident and not Miranda. Oh and I forgot Rebecca and Alex went their separate ways as did Ales and Addison and Addison and Mark. Nobody dies but it was a lot of changing in that single episode.
Otherwise, I would go with Meredith dying.
I gotta agree with the ep where Izzie cut Danny's Lvad wire the close second.
