when he told him he spent 40 days with somebody on carnival and house knew that the carnival in bolivia was only 8 days, so he determined it was the wrong location. i think thats what happened.
The other person is correct. House knew it could not be Bolivia when the agent said carnivel was 40 days. He explains the nuts part very fast. He said that in Brazil they call Brazil nuts just 'chestnuts'.
He tells House that he spent forty days of Carnival with some female government minister. House realizes that Carnival in Bolivia only lasts eight days, and the guy said he spent 40 days there. The guy was in Brazil, not Bolivia. He ate too many Brazil Nuts. The selenium in the nuts caused his illness (too much selenium poisons you).
