Some of the creatures they let out were out hunting. Their first encounters with these horrors were the " 7 Deadly Sins", which took possession of bodies and killed according to the sin- sloth, envy, gluttony etc. Sam was still trying to find ways to get Dean out of his deal with the demon.
Sam almost became a victim of the " Deadly Sins" but he was saved by a beautiful blonde who can kick butt and promptly disappeared.
