um... the episode was called Closure in the 6 season
they found Samantha's diary which clearly stated that she was tortured and then died, while the credibility of the man who brought the agents to this house was questionable, Mulder seemed to believe the diary and found peace, he believed Samantha died
we're not sure whether it's true or not but after that there was no mentioning of Samantha in the series
They "found" her and lost her again in a couple of episodes; the question was, was it really Samantha?

All sorts of Samantha Mulder were discovered: a set of younger clones, a spirit version, and the real Samantha, and a few others. Samantha never came back to live in DC, though.

No, she was found to be held on a "farm" where other people who disappeared were brought by the Government. I remember seeing Mulder finally seeing her but she was not allowed to leave.
