In this song i think it has to do with a lifestyle or a choice made by someone!.
"I dont think you trust in my self-righteous suicide" Means someone doesn't believe the person is doing the right thing, but the self-righteous guy is doing the right thing, even if it looks like suicide to everyone else!.
Self-righteousness is confidence in ones own actions(he/she is doing the right thing), especially against the will of others!. It relates to the whole song because Serj sings about putting on make-up and hiding the scars in one verse, but then talks about self-righteousness in the next!.
