I know what you mean... It happens to me every time I see one of my most favorite movies: "The Silence of the Lambs". When Clarice is going to see Dr. Lecter and on the way there through that dark kind of tunnel, she sees and hears the inmate next to Dr. Lecter telling her "I smell your..." But, the worse part is when I really kind of gag and want to vomit, is when the same inmate throws at her his... and hits her "on her face!".
Man! It takes lots for me to make me gag but that does it!

The worst scene I've ever seen in a movie was probably the infamous wood chipper scene in Fargo.

The rape scene in Irreversible. I couldn't even look, it was sickening, and oh so real.

It's between the scene in Shutter when the dead girl corners the guy and sticks her 6-inch-long rotten tongue down his throat, OR in 28 Weeks Later when the guy is infected with the rage disease and jumps on top of his wife's hospital bed, beats her up and then rips her eyes out.

In Pan's Labyrinth, the part where the step-father gets his cheek slashed open by the rebel woman is just disturbing. It flaps when he talks and then he proceeds to sew it up.

Pretty much any scene from Cannibal Holocaust...such a gross movie

A scene that I can never get out of my mind is the one shot by David lean in " The Bridge on the River Kwai " The entire film is shot in the jungles of Burma, but in one place the scene shifts to the lush greens of Colombo, where the Captain is beinfg briefed and commanded to return to the war theatre in Burma from where he has just escaped & destroy the very bridge that he had been forced to build ( by the japanese as a prisoner of war.

This particular shot of Golombo is a visual beauty - a treat for sore eyes. It is also a turning point in the movie. Even as Sri Lanka is struggling to come to terms with her ethnic war and lies battered today, the erstwhile beauty of the island, as depicted in this scene, just refuses to go out from my memory.

If we're also talking about based on true life, then in 8 Seconds at the end where they show actual footage how Lane Frost died from a bull goring him - the bull tosses him around like a rag doll!

EDIT: Agree with those above for American History X where he stomps the guys' head into the curb. I have to look away too
