If the picture is a dead animal or child etc or something really sexually screwed up then I usually do not worry about the avatar.
Most of the time i don't even notice the avatar or the user name. if we are here to answer questions, then those things don't matter.
Whether or not you choose to answer the question is totally up to you. If the avatar is very offensiveyou can check to see if it is against Yahoo Answers Regulations and Guidelines and report it if it is. I reported one not too long ago for being very sexually explicit.
