I always start-up a conversation. It's practice for networking and talking to women when I'm out.
Depends if I am on with a bunch of hot chicks or a bunch of shady looking characters.
I inform them that I am button happy, so I ask everyone what floor they are going to, and then I choose to have a conversation with a couple of them :)

I talked to them (it usually makes them very uncomfortable and I enjoy the weirdness of it.) I asked them random questions (it all depends on where the elevator is)
I avoid them by looking at the ceiling or at the door saying "come on, come on, when are you gonna open up"? Esp. when there are ppl in there with body odor..UGH!
Depends...usually I avoid all eye contact (just like they do) or if it is really bad weather or a long wait for the elevator people strike up a quick conversation about it.

I find it impossible to think of something to say. You don't know how long the ride's gonna last or who's gonna get off when. I make like a monk and take a vow of silence.
I say, "Hello", flash a polite smile, THEN commence to fidgeting and messing with my phone...I have an awkwardness/anxiety around strangers :(
It depends, if I'm happy and high I'll strike up conversations if I haven't had my coffee and not so giddy I'll play with my phone.

I just stand there and stare at the door or the buttons. I'm in an elevator every 20 minutes, and only for about a minute at a time, and I think it's awkward when people try to start a 2 second conversation...
I work in a 15 story office building that is occupied 90% by lawyers...so when I get in the elevator most of the paralegals act snobbish and most of the lawyers are on their cell phones...so I avoid them.
