My take: The Back Door Man takes care of all the ladies in town!. Although it is not stated, we would assume that the man who shot the back door man was the spouse of one of the ladies BDM is "taking care" of!. BDM shoots back and kills the man!. The nurse, judge's wife, cop's wife don't want BDM convicted because he "takes care" of them!.

the lyrics arent especially clear, but i had assumed he was in a shootout, he ended up in the hospital, shot full of holes, but he killed the other man, murder in the first degree!. and thats why the cops wife tries to prevent the execution later!. Www@Enter-QA@Com
