The song and video are based on a book called "Johnny Got His Gun" by Dalton Trumbo. Much of the footage in the video is derived from the very dreary 1971 movie of the same name. The movie is actually quite difficult to find, I seem to recall. Metallica actually bought the movie and all rights to it outright. I have just learned that a re-make is due to be released some time later in 2008.
The story is about a man who goes off to war and is very seriously injured. He loses all his limbs and senses - and is essentially cut off from the world. In the book the only sensation he has is touch - usually from his nurse.
The doctors consider him to be de-cerebrated - essentially brain-dead. He just lies there alone with his thoughts.
It's a very depressing story.

The song and video are actually based on an old movie. A man is in the hospital b/c he has lost almost everything, except his brain! He can't walk, he can't talk, he can't move. He can't see, he can't hear. He can only comprehend what is going around him but can't communicate with anyone. He is "one" , he is all alone and eventually your mind starts playing tricks on you. He doesn't know if he is dreaming or the things he is seeing in his mind are actually true. He just wants to die b/c living is a slow painful hell. He is like this b/c he was in war and was injured via a landmine.
