Answers: Do you play the high E string or not?
a. No, you leave the top E out of it.

b. Use the first three fingers (index, middle and ring fingers, i.e. 1, 2 & 3. )There are some add-ons to the standard A chord which involve using your pinky (little finger or 4th finger) to add an extra note, so you need to keep it free.
EDIT: Most of the time, you will be extending the 4th finger to another fret, while keeping the A chord 'locked" with the first 3 fingers. So develop strength in that finger, it comes in useful!
Bowzer's right. If you can get in the habit of barring all 3 strings with your ring finger instead of using the 1, 2, & 3 fingers, it will help prepare you for A-shaped barre chords (you'll need this later).
Best of luck to you!
