"Addicted to Vaginal Skin" by Cannibal Corpse is a brutal affair. It begins with this real sound bite:
"I don't know, I just took that knife and I cut her from her neck down to her anus and then cut out the vagina....and ate it"!
'Bullets Are Mine" by Grave is about nailing someone to the floor with a gun. Some lyrics: "Murder, another D.B. found. Body Nailed to the ground. Died on his knees, on every count I am guilty"
But I think the most brutal/sickest song I know is "Mein Teil" by Rammstein. This is a song about a true event. Two men met over the Internet and agreed to meet so one could kill and eat the other one. The first thing the man ate was the other man's penis. The words Mein Teil translated into English means "My Part".

I would have to say the most brutal that comes to mind is the song "Kim" by Eminem from his Marshall Mathers LP. Other than that, I've heard many brutal songs and bands, but that song used to give me chills.
Not sure which definition of brutal you are looking for, but the song 'Thunderhorse' by Dethklok might fit both definitions.
