I'm not sure it's about permission, as much as its about the artist not getting paid to promote the brand.

For example, if I was Nike, I'd LOVE to be on a T-shirt in a popular rap video... But if the record company comes by and says "hey Nike, we have your logo on our artist during his video... pay us $1M or we're gonna blur it out" then I'd say "blur it out... you're not getting $1M from me"

probably do not have permission to advertise certain brands. i would be a little angry if some dumbass "thug" wannabe was sporting my cloathing line, or product all over television, agree?!

usually it's a 'make' of clothing. EG- Hurley, Adidas, Nike etc, when they don't have permission too show the brand on that particular thing. (video,program)

Most of the time it's just a name brand logo that they do not have a license to use.
Sometimes they get the license and it's great advertising for the company. Usually pop singers advertise this way ( Think Jessica Simpson and the tune in her video)
Depending on who the rapper is, they could be blurring gang affiliation signs and curses. If it's that, you can see it when the video is played late night.

MTV doesn't allow advertising, they won't even allow you to say slogans, or show logos.

the clothes have names of brands. they aint allowed to show them in the videos
