He's dying of cancer!.!.!.he wants people who aren't dying to appreciate their lives!. So he gives them choices, and if they do it and survive, then they've learned their lesson, and will live their lives right!.
The girl in the mask, is a girl who survived, and learned the lesson or whatever, and was helping him, but the second time she got tested she failed and was killed!.

The whole point is these people somehow abused themselves or others (such as sold drugs or tried to kill themselves!.!.!. basically somehow did wrong) and Jigsaw is trying to get them to better their life and realize that its precious and matters so much!. He's doing this (as it explains in 3 or 4 I think) because he tried to kill himself by driving off a cliff but ended up surviving it realizing how important life is!.
The person that took is place is that cop!. Not sure of his name!. The pig mask girl was to but she wasn't doing it right and they offed her!.

Ok soooo the guy has cancer and is a complete psychopath so he does it for pure enjoyment!. Plus he also wants to make people value their life since it can be taken away from them in an instant (the whole cancer thing)!.
