No, you won't have any trouble understanding it!. I wouldn't be a waste of time, though, to watch Batman Begins because it is also a very good movie!. You should be able to rent it pretty cheap now because in is a few years old!. It goes into a lot of detail about who Bruce Wayne really is and why he becomes batman in the first place!.
To answer your question- you will be able to understand The Dark Knight, but I think you could get more enjoyment out of it by watching Batman Begins

I didn't see the first Batman movie, but I had no trouble at all understanding "Dark Knight"!.
By the way: "Dark Knight" is AWESOME!!
