Because in part of the movie hes reading the magazine and theres a article with letters from peoples organs and stuff and one of them said "I am jacks colon!." or something like that, and then tyler said "I am cancer!. I kill jack!."

He mentions it in the movie!. He is referring to a medical text he saw in which diseases are personified!. "I am Jacks swollen prostate!." It is just a cool way to bring continuity throughout the whole film!.Www@Enter-QA@Com
