I saw this movie ages ago!. The answers to your questions are below:

1) Adam Carr (David Boreanaz) was Jeremy
2) Dorothy didn't do any of the killings!. If I remember correctly, Jeremy (Adam) just put the mask on her so everyone would think it was her and he could walk away!.
3) Why!? Because Jeremy (Adam) wanted his revenge!.
If you remember, his nose bled when he was a kid and his nose bled at the end of the movie!. It bleeds everytime he's nervous/embarrassed/worried!.
He left alive the only girl was nice/wasn't mean to him (and fell in love with her)!.
