Exorcist - especially the re-released version with the crab walk.
Alien - I still hold my breath when the alien baby bursts from that guy's chest
Silence of the Lambs - tension builds throughout until that terrifying night vision goggle scene at the end of the movie.
Scariest? My top three would be Amityville Horror, White Noise, and I am Legend.
This doesn't match my view on best scary movies of all time but I'd definitley have to say they're the scariest for me. They all give me trouble sleeping. I still watch them all the time, though!
I'm not a big horror movie fan. The scariest one ever for me was "The Blair Witch Project."
i don't know about all time, but i would recommend any or all of the Saw movies for a good scare.

I realize this isn't scary at all to most people, but I will never be able to watch it alone lol. I can any watch scary movie you can throw at me, but this one scares the **** out of me.

Jaws (didn't go in the ocean for a year)
Suspiria (Dario Argento's masterpiece)
In the Mouth of Madness (a complete trip that just unsettles)
