I suspect that you're thinking of a scene in Little Shop Of Horrors, in which a character enjoys the pain caused by going to the dentist. (In the low-budget, 1960 version of the film, the part of the patient was played by Jack Nicholson.) There is also a musical version, which came out in 1986; in that one, the patient is played by Bill Murray.
The only other film that I can think of is Novocaine, but that's more of a comedy than a horror film. It is part mystery, but still, it's played more for laughs than for shrieks of horror.
