They didnt have cancer the virus cleared that problem up. It just mutated their bodies to be unable to stand UV light and all the other things that they couldn't be exposed to. The book explains it a lot better but I haven't gotten too far into it. Essentially they are supposed to be equivalent to vampires but in this movie they were unable to speak like in the previous installments and the book. These ones seemed almost completely animalistic except the guy with the dogs. The thing I did not like about it was the way it ended though he does die in the end of the story but there was a lot left out of it. Read the book its pretty good. Its by Richard Matheson, it provides a better look into how the time alone effects him and makes it a lot more creepy.

If your worried about that happening to us here a thought. The disease that stuck the world was related to rabies but in real life two animals with rabies would attack each other instead of building up clans that feed only on non diseased species. So if some plague equal to to movie were to strike and a new form of rabies were to come over the world the number of monsters portrayed in the movie would be significantly reduced because they would kill each other off first.
