The man in the wheelchair is not Hannibal's shrink, he was one of his early patients, Mason Verger. Mason describes himself as "the one that got away", because he's the only one of Hannibal's victims that lived. But Hannibal apparently wanted him to remain alive. Hannibal drugged Mason, and convinced Mason to slice off parts of his own face and feed it to his dog -- hence Mason's desire to feed Hannibal to the pigs.

Paul Krendler, Starling's FBI supervisor, is the man who gets his head sliced open. The brain indeed has no nerve endings -- neurosurgeons can operate on a patient who is conscious if they so choose. He gets fed to himself, of course, and is killed by Hannibal before he leaves the house (in the book, anyway -- that's not made clear by the movie).

Personally, I found the ending of the movie a complete copout and would have preferred that Ridley Scott stayed faithful to the book's ending, with Hannibal and Clarice running off together. It's a twisted and horrifying vision to think that Hannibal's personality and force of will could be strong enough to poison Clarice's mind in such a way.

A, the guy with the f-ed up face was Hannibal's original shrink; he and Hannibal were going to (or already had had) have sex, when Hannibal sliced him, burned him, etc.,.
B, THE BRAIN HAS NO NERVES, SO THE GUY WOULD ONLY FEEL A BIT OF MOVEMENT OR PRESSURE AS HIS BRAIN WAS BEING CUT; HE WAS PROBABLY DOPED-UP/KNOCKED OUT WHEN THE CUTTING WAS DONE TO SEPARATE THE TOP 1/2 OF HIS HEAD, THEN SUBSEQUENTLY LOBOTOMIZED, DOPED OR IN SOME OTHER WAY TRANQUILIZED AFTERWARD. Yes, he could live that way for a long time.

I was sorry that Hannibal didn't get fed to the pigs. And I felt badly for the Italian cop, but hey! he got greedy.

I am going to agree with the first guy however do you remember in the flash back scene when that guy was druged up and he cut himself with a piece of glass? Ya that would explain the face but I was always confused as to why he was in the wheelchair. Also yes he was very druged up and at that point would not feel anything. Simple commands like say hello. It seemed like a while but it was not really, it was for effect.
