Just depends, A lot of the time they will film in old prisons. You know, prisons that have been closed down and what not. The movie The Longest Yard was flimed in an old run down closed prison in Texas.

most are real
in Shawshank it was a prison that was closed down (Mansfield Ohio) and they filmed it before they tore it down
usually they get permission to shoot in a prison like they did with the Blues Brothers..opening scenes was Joliet Correctional ..the same one was used for the first season of Prison Break, and the closing scene it was Cook County Prison
some movies just make up a set ..but if you want real affects they use shuttered prisions most of the time

I'm willing to guess that most interior shots are on sound stages. Some exterior shots may be in closed prisons that now operate as tourist traps.

If you look at a lot of these movies, most just have a block wall behind or store, or a wire fence: easy to fake.

Well, sometimes they use old prisions. But most of the time, they just make a prision set.
