In that scene, the townspeople are running to the church to find safety from the coming "darkness". The "darkness" is a very industrial, hell-like world where horrifying creatures roam and, often, kill anyone and anything that gets in their way. One of the townspeople, a girl by the name of Anna, is distracted by Dahlia (another person in the town) and tries to throw a rock at her. While the girl is preparing to throw the rock, Red Pyramid/Pyramid Head (one of the creatures) forms behind her, grabs her by the back of the neck, and, after ripping the clothes from her body, rips the flesh from her body (and then flings it at the doors of the church as they close).
