The girl from "Poltergeist" died after filming for Poltergeist 3 was finished. Only one person has ever said that a person other than Heather O'Rourke was used in filming the ending (and he only said it was a body double, not Drew Barrymore).

Nick Papac, he wasn't as actor but he got killed on the set of "The Kingdom".
And even though Nicole DeHuff didn't die while in the process of making a film, she was in Meet the Parents and was supposed to be in Meet the Fockers but couldn't because she got pneumonia.

Heather O'Rourke is the famous poltergeist star who passed away during the production of Poltergeist 3. I love those movies!
Massimo Troisi, the actor who played the role of Mario Ruoppolo in "il Postino" (1994), died 12 hours after finishing the filming. He had a heart condition which required surgery and he posponed it until finishing filming the movie.
By the way, if you haven't seen it yet, I highly recommend it. It's a beautiful movie.
