Yes, he got a lobotomy. Nurse Ratched ordered it.

He was zombie-like because every patient who receive a lobotomy loses a part of his/her brain and he will become a vegetable. Lobotomy patients also won't know what's going on around him/her. After a few months after the lobotomy, the patients will have a VERY different personality (because of the lost brain lobs), that's why The Chief killed him...

If the Chief didn't kill McMurphy, Nurse Ratched would be victorious and by killing Mac, other patients don't have to see Mac like that and he would be "released free". Of course The Chief can't take Mac with him, can he?

yes it was a lobotomy. the procedure removed parts of his brain which they felt controlled his aggressiveness and his ability to think. after removing those parts, he was nothing more than a vegetable. sad really..

in the end of the movie Mc Merphy was indeed labotomised the scars are from the sergery.
