I would have to say The Exorcist. I first watched it when I was 21. I know it's kind of cliche, but the fact that it is based from a true story FREAKS ME OUT!
I watched the Texas Chainsaw Massacre(the original one) when I was 6 years old and that scared me pretty badly. I live in Texas not more than 30 miles from where it happened at.
The original day of the dead also was pretty scarry.

It's funny when I think about it now but when I was 4 the scariest movie I ever seen was the Blob. It was scary because it reminded me so much of Jelly and there was jelly in my fridge. I thought it was gonna crawl out and attack me. I couldn't eat the stuff for like 3 weeks.
the silver bullet I was 6 watched it early morning while it was dark out scared me then now its really lame
when i was when i was 6 i saw the Blair Witch Project and every night i couldnt sleep for like a month, i thought the blair witch was going to kill me lol
Halloween 8 i was like 10 and couldnt sleep i feel asleep for my a minute or two but woke up because i thought he was coming for me and woke up with huge bags under my eyes and very tired lol.
Skeleton Key was super scary. I don't know, just something about all the chanting and magic stuff that freaked me out. I saw it in the theatre then bought the DVD, but have not yet had to courage to watch it again.
