the mobster that's "trying to have Israel killed" is really an undercover cop who got lots of plastic surgery to live on the inside in the world of crime and help police. There fore he is working with the police to capture Israel and take his heart because he has a fatal condition and needs a heart transplant to survive. Israel is his son so he is the only person that they could take a heart from for the transplant to be a success. In the end that main youngish guy finds out everything and he is so pissed that all of those innocent people, including his older partner, died just to save one person (Israel, well in the end actually his dad) that he pulls the plugs on both of them when they are asleep in the hospital at the end of the movie. It's a very dramatic ending. You should probably just watch it for yourself...

The old guy that is trying to have him killed isnt actually trying to kill him. Israel is the old guys son and he wants to have him heart transplanted into him.
