It is not stated exactly, only that Catherine died two years or three ago and Garrett has been morning her ever since. The bit about cancer might make senes being as Catherine was pregnant at the time and probably would not have been willing to abort the pregnancy if she thought her cancer was malignant. The fact is that the story is semibiographical and it is about Spark's father and mother. Spark's mother died falling of a horse and it took his father four years to recover from the loss. Catherine is actually Spark's wife's first name. In real life, Spark's father died just like Garrett having found a new love but almost immediately dying thereafter. The pregnancy thing was in there for a double whammy, but it is possible Catherine died having a miscarriage . Like I said it is not certain in the movie or the book. Catherine was sick for a long time with cancer, she felt the end was near and went to the inlet surf to put a bottle in the ocean with a message. 1of the 3. It was cold and storming out . When they found her she was out their to long, pneumonia set in and she didn't recover, she died. If she did not have the cancer she probably would not have died of the pneumonia. Hope I helped
