i had the same feeling. but i get it now. here we go:
the autopsy scene that opens the movie (and at the end) is the newest scene in terms of chronological order. the rest of the movie is a "flashback." i put it in quotes because it is really just a further explaination of SAW III. Briggs's game and Agt. Perez & Agt. Strahm (Scott Patterson, dude from Gilmore Girls) investigation is happening simultaneously as SAW III 's events. As the movie shows, Agt. Strahm is in the warehouse that is the setting for SAW III and gets to the room where all the action took place and Jeff is still there.
Dectective Hoffman (guy from the electric chair with Detective Matthews) was Jigsaw's accomplice for Jeff's game (from SAW III) as he helped Amanda and Jigsaw setup for that game.

the autopsy scene. as i mentioned, this is the most current in terms of chronological order. the tape covered in wax that Jigsaw swallowed at the end of SAW III was directed at Hoffman. It is saying that Hoffman will now be tested as he will put in a game. The stage is set for SAW V. (although no one knows who will be continuing Jigsaw's work.)

I saw it today too! I was also really confused. What I got out of it was this movie was going on the same time Saw III was. So detective Rigg was walking through the house at the end the same time the father from Saw III was. At the end of the movie, the tape recorder tells the detective in the electric chair "you think you are in control now, but you now will be tested".... which I believe is what the tape recorder in the stomach of John was saying. I dont know if you could follow that....
What really confused me is that two of the detectives looked A LOT alike. I got them confused so much I had no idea what was going on.
