John Kramer, Jigsaw, is dead, his body recovered by authorities and subjected to an autopsy. However, during the operation, a tape is found in Jigsaw's stomach, coated in candle wax. Detective Hoffman is called to the scene where he plays the tape and is told that Jigsaw's work will continue despite his death.

The scene changes to show two men in one of Jigsaws traps, attached by the neck to chains which pass through a machine. One victim is rendered blind, his eyes sewn shut whilst the other is mute, his lips sewn together. In a panic, both men assault each other, the mute man gaining the upper hand and killing his opponent before using a key chained to his neck to free himself from the trap.

The police meanwhile have uncovered a trap and the remains of Detective Kerry, her ribs torn from her chest. Rigg, distraught at the find, leaves the scene as FBI agents Perez and Strahm arrive, sumising that Jigsaw had help in concocting this particular trap and that the help was from someone other than his apprentice, Amanda Young.

That night, Rigg is attacked in his home and left unconscious, awakening to a new Jigsaw trap taking place in his own home. Rigg struggles to free Brenda from the trap while Jigsaw advises him that she is a criminal and undeserving of life, disclosing her life as a pimp of young girls. Rigg frees her but not before she suffers severe damage to her torn scalp, however, after she is freed, her 'test' begins and she attempts to kill Rigg before being subdued. Finding clues to the next phase of the game, Rigg travels to a particular hotel and uses a key supplied by Jigsaw to gain access to a room that contains his next instruction. Rigg captures the next victim, Ivan, and moves him to another room which contains another trap. Surrounding by photos and video evidence of Ivan's sordid past, an enraged Rigg forces him into the trap to let him pay for his crime. Rigg reads his next clue while Ivan, taking too long to complete his task, is torn apart.

The scene shows a flashback of Rigg's past, before he became a member of SWAT, dealing with the apparent beating of a child by her father. Unable to confess the crime to Rigg, they are forced to let the man go but not before his comments force Rigg to assault the girls father. Rigg escapes prosecution by lawyer, Art Blanc, thanks to Hoffman's falsified report.

The FBI raid Rigg's home, believing that he may be connected to the Jigsaw killer. There they find that the Brenda has died from her injuries. The evidence there leads them to suspect Rigg and also helps them to discover Jill, Jigsaw's ex-wife, as a possible source of information about the recent traps.

In current time, Rigg enters the now derelict school where the event took place, finding the deceased abusive father and his wife, chained together with a single spike piercing their body. A flashback shows the women being instructed to escape her abusive relationship by removing the spikes from her body while simultaneously killing her husband. However, in the present, she has been unwilling to remove the final spike. Instructed by Jigsaw that Rigg must stand aside and allow her to complete her own test, Rigg hands her a key which will unlock her from her shackles but tells her that she must remove the final spike herself as he follows his next clue but not before pulling the fire alarm so as to alert the authorities.

As the FBI and police investigate the crime scene, an accident with the now-completed trap occurs, spearing the forensic photographer through the neck and killing her. Frustrated and disgusted, Perez and Strahm search the school further, finding Billy the Puppet and a tape addressed to themselves. Perez is told that Strahm is about to kill an innocent man and that her next step is crucial. Peering closer to investigate a sound, the puppets face explodes, poisoning Perez and revealing a camera hidden in the puppets skull.

Desperate, Strahm attempts to illicit information from Jill. Flashbacks reveal that Jill lost Johns and her baby due to the actions of a drug addict named Cecil. John was sat outside the building, waiting to pick Jill up where he was solicited for sex by Addison, who would later become another of his test subjects. Noticing Cecil running from the scene, John investigates, finds Jill wounded and carries her to the hospital. The baby is lost and John is visibly distraught.

John becomes obsessed with the idea that only the victim can change themselves and begins developing traps to test subjects. Jill, having healed from her assault, comes to find John with Art Blanc to discuss a project that he is partners in. John shows no interest, still consumed with his loss and tells them both to go away. John tracks and abducts Cecil using plastic pig masks as a disguise. Placing Cecil into a trap, John explains that he wants to make Cecil as ugly on the outside as he is inside, engaging him with the 'knife trap'. Cecil must force his face through a mesh of knives to push a button to free himself or he will bleed to death from blades that are penetrating his wrists. As he struggles however, the trap breaks and Cecil is freed. John, unwavering, goades Cecil to attack who does, missing John and lunging himself into a pile of barbed wire that kills him.

Moving back to current time, Rigg has followed his clue to the Gideon factory with Agent Strahm arriving close behind using information obtained from Jill. As Rigg and Strahm move through the facility, we see Jeff Reinhart, subject from Saw 3, also working his way through the building. Strahm follows the noise that Jeff is making, leading him away from Rigg while Rigg finds the door that will lead to his final test.

Detective Matthews, Detective Hoffman and Art Blanc reside on the other side of the door, each involved in a test. Matthews is revealed to have been held prisoner by Jigsaw for 6 months, given an iron cast to compensate for his shattered leg. Art, revealed to be the same man in the neck-chain trap, has been instructed that once the time for the test is up, he can activate a switch which will free them all. Part of the test requires him to give Matthews a revolver with a single bullet. Each subject discovers however, that if someone enters the main door, they will trigger another trap which will instantly kill Detective Matthews. An exhausted Rigg lunges at the door, smashing through it as Matthews shoots him in a vain attempt to stop him. The trap is activated and Detective Matthews is killed, his head crushed. Rigg shoots Art believing him to be Jigsaws accomplice, fatally wounding him. Rigg is told that he has failed his test, his obsession to save others has inadvertently led to their death. Had he simply allowed time to run out and not saved them, they would have been safe from harm and released from their traps.

Meanwhile, Strahm follows Jeff into a sealed room where he finds Jeff and the bodies of Lynn, Amanda, and Jigsaw. As Jeff waves his weapon in a threatening manner, demanding to know the location of his daughter, Strahm kills him in defense.

As Rigg lies wounded on the floor, Hoffman releases himself from his trap, revealing that he is involved in the game, a new Jigsaw apprentice. He leaves Rigg to die, stating "Game Over" before sealing Strahm inside his room. It is revealed that the events of Saw 4 have run concurrently to those of Saw 3, with Strahm intercepting Jeff just after the death of Jigsaw, Amanda and Doctor Lynn.

The autopsy scene that occured at the beginning of the film is shown again, revealing it to have occured after the events of the film and not before. As the tape found in Jigsaws stomach is played again, Detective Hoffman is told that the games have only just begun.

John Cramer does not come back to life, he had died.

But SAW IV takes place the same time SAW III does. Like the person above said, the whole SAW IV is a flashback. Its hard to understand, but once you get it you will enjoy it.
